Subject: Forms issued for income tax purposes
We would like to remind you that in conformity with the Financial and Administrative Services Policy 2.3.1a T4 “Statement of remuneration paid” for Canada Revenue Agency and a Releve 1 “Employment and other income”, for Revenu Quebec, will be issued by the end of February 2012 for income tax return purposes.
As per the Churchman VS her Majesty the Queen, Chairpersons and Board members may claim certain expenses for tax purposes. If this is your intention, you need only request that the Board centre manager, where you are appointed, to provide the completed T-2200 form (Declaration of Conditions of Employment), and for Québec members the TP-64.3 (General Employment Conditions).
Please wait until you receive your T4 (Relevé 1, in Québec) before requesting it, via the Board of Referees assistant, as these documents go hand in hand.
